Website, Portal, Email and Phone Shutdown ~ September 12, 2014

In preparation of our corporate office relocating to a new office in Easton, MD, our data center will also be relocating to the new facility on Friday September 12th, 2014. Please be advised that all IT services including all telephone and all systems connectivity will stop at NOON on Friday, September 12th 2014. Additionally, there will be no IT services during the weekend of September 13th & 14th 2014. Our Florida staff will still be available via their cell phones.

This outage will also include this website and all SharePoint portal sites!

Our IT systems will be restored sometime on Sunday, September 14th 2014 and we will restore all operations on Monday, September 15th 2014.

Delmarva Foundation

The Florida Developmental Disabilities Resources Website is a public reporting resource for individuals with developmental disabilities living in or moving to the State of Florida. The primary purpose of this Website is to help individuals with developmental disabilities find a provider who best meets their needs.

Delmarva Foundation, a certified Quality Improvement Organization (QIO), works in partnership with the State of Florida’s Agency for Health Care Administration (AHCA) and the Agency for Persons with Disabilities (APD). It is important to note that funding for Delmarva does not reduce the amount of money APD has for services or the amount of services an individual may receive.

A primary goal in the Florida service system is to improve the quality of supports for Florida citizens with developmental disabilities.  The values supported and actively promoted within the system are:

  • choice,
  • full inclusion,
  • health and safety,
  • respect and dignity,
  • self-direction, and
  • satisfaction with all services and supports.

Provider performance information is available for providers who have received a “Provider Discovery Review (PDR)” from the Delmarva Foundation in the past 18 months.  Performance scores reflect waiver rule compliance based upon the number of standards reviewed during the Provider Discovery Review. Visit www.dfmc-florida.org for a brief description of the review process.

Information for providers who have billed for Medicaid Services in the past 12 months is available through these websites for the following Developmental Disabilities Home and Community Based Services (DD HCBS) programs:

The information includes: provider name, contact person, address, telephone number, services provided, and location. Additionally results on several performance review areas are reported for each provider..
