Welcome to the Florida Developmental Disabilities Resources Website

Notice: This site is under construction and information will be updated following meetings with AHCA, APD, Delmarva and Stakeholders to decide the content and format.

The Florida Developmental Disabilities Resources Website is a public reporting resource for individuals with developmental disabilities living in or moving to the State of Florida. The primary purpose of this Website is to help individuals with developmental disabilities find a provider who best meets their needs.

Delmarva foundation, the quality assurance organization, works in partnership with the State of Florida’s Agency for Health Care Administration (AHCA) and the Agency for Persons with Disabilities (APD). It is important to note that funding for Delmarva does not reduce the amount of money APD has for services or the amount of services an individual may receive.

A primary goal in the Florida service system is to improve the quality of supports for Florida citizens with developmental disabilities.  The values supported and actively promoted within the system are rights, choice, full inclusion, health and safety, respect and dignity, self-direction, and the person’s satisfaction with all services and supports.

Contact information for providers who have billed for a service in the past 12 months is available for the following waivers:  the Medicaid Home and Community- Based Services Waiver, iBudget Waiver and the CDC+ Program (http://apd.myflorida.com/cdcplus/),   The information includes: provider name, contact person, address, telephone number, services provided, and location. Results on several performance areas are reported for each provider. 

Provider performance information is available for providers who have received a “Provider Discovery Review (PDR)” from the Delmarva Foundation in the past 18 months.  Performance scores reflect compliance based upon the number of standards reviewed during the Provider Discovery Review.  Go here (www.dfmc-florida.org) for a brief description of the review process.
